Behavioral Health Consultant

The Clinical Social Worker is responsible for working with the primary care team at the Beaverton clinic to offer comprehensive, in-depth social work interventions to address the biopsychosocial needs of patients and families.
The general goal of intervention is to encourage enhancement of the patients physical/psychosocial functioning through direct treatment and coordination of facility resources.
Provides patient and/or program consultation and r5eferral to community agencies.
Our service provides care for adults across the lifespan.

Behavioral Health Counseling Detect and addresses broad spectrum of behavioral health i.
e.
, mental health, substance use, complex psychosocial needs, grief support, mandatory reporting issues); performs triage, warm hand-offs, risk assessment, brief psychotherapeutic counseling intervention and care management services.

Performs Psychosocial Needs Assessment Complete bio-psychosocial needs assessments to help determine appropriate level of care.
Work with patient & available support systems to develop appropriate treatment plans/meet goals of care.
Consultation with facility staff or other community professionals seeking guidance in meeting patient needs.

Coordination of services Provides information and referrals with OHSU partners (i.
e.
Financial Specialist, RN Clinical Specialists, Patient Advocates and other professionals as appropriate).

Community Resource Referral and Psycho-education- Completes education and assessment of social determinants of health, social/emotional implications of medical situations, guardianship or conservatorship, and/or other high-risk needs.

Provides information about Social Security Disability and other financial resources as appropriate.
Acts as advocate to refer patients to appropriate services within the community.

Collaborates with clinical staff to coordinate and gather pertinent information from patients to support clinical management of chronic conditions.

Increases individual patient engagement, capability, and accountability through regular patient contact.

Provides education, information, and encouragement to patients about available health interventions including diet, fitness, behavioral and social connectivity programs.

Actively manages a patient panel.

Education:
Masters degree (MSW)
Job Related Knowledge, Skills and Abilities (Competencies):
Experience in behavioral health counseling is required; knowledge of the principles and practices of behavioral health intervention, including assessment, diagnosis, treatment planning, and psychotherapeutic counseling modalities.

Comprehensive understanding of the interrelationship between medical and psychological systems and their impact on health.

Experience working with an underserved adult population experiencing mental illness, substance abuse, complex psychosocial needs and chronic illness.

Registrations, Certifications and/or Licenses:

Masters Degree in Social Work
Licensed Clinical Social Worker (LCSW)
Or Clinical Social Work Associates (CSWA).
Education:
LCSW
Experience:
Two years in behavioral health field/healthcare related field.
