Summary:
As the Community Nurse you will be responsible for administrative duties as well as providing hands-on care for our residents.
You will be part of a dynamic well lead team of professionals including our Director of Health Services, Community Nurse LPN, and our Memory Care RN.

Responsibilities:
Input new resident data into the system and update current resident's record.
Participate in care conferences and developing resident's Service Plan.
Coordinate care with outside providers and maintain appropriate documentation.
Respond to resident/family concerns identified and reported by Caregivers and Med Techs.
Document directions and actions appropriately, e.
g.
, chart notes, nursing orders, and communication log Perform direct nursing care as needed Participates in providing continuing education to staff, as assigned.
Member of CQI (Continuing Quality Improvement) team.
Investigate, evaluate, and document incidents or accidents.
Work collaboratively with other care staff and management team.
Must be self-motivated and able to think independently.
Update ADLs and communicate changes with physicians and care staff, and families as needed.
Follow-up on lab reports and doctor orders, as per policy.
Respond appropriately to emergency situations.
Rotating on-call Other duties as assigned.
