Home Health Registered Nurse

In this role, you will have the opportunity to provide high-quality, personalized care to patients in the comfort of their own homes.
Responsibilities:
- Conduct patient assessments and develop individualized care plans in collaboration with the interdisciplinary team.
- Administer skilled nursing care to patients in accordance with physicians' orders and established policies and procedures.
- Provide wound care, medication management, and other nursing interventions as necessary.
- Educate patients and their families about their conditions, treatment plans, and self-care techniques.
- Monitor patients' progress and adjust care plans as needed to promote optimal health outcomes.
- Communicate effectively with patients, families, and other members of the healthcare team to ensure continuity of care.
- Document all patient interactions, assessments, and interventions accurately and in a timely manner.
Qualifications:
- Current license to practice as a Registered Nurse in the state
- Minimum of one year of clinical nursing experience, preferably in home health or a similar setting.
- Strong assessment, critical thinking, and communication skills.
- Ability to work independently and prioritize tasks effectively.
- Reliable transportation and valid driver's license.
- Willingness to travel to patients' homes in the Portland area
