Registered Nurse - Complex Case Manager

Description: Providence is calling a Registered Nurse - Complex Case Manager to Providence Health Plans in Beaverton, OR. In this position, you will: + Provide care coordination services to Providence Health Plans (PHP) members, including: Disease management programs, in Educating, Motivating and Empowering members to manage their disease. + Provide Case Management services including: triage and referral, transition of care planning, end of life care planning, other acute and catastrophic cases. + Provide Care Management services including nurse education, care coordination and general assistance with managing day to day functional needs; assisting with the management of member health plan benefits and offering assistance finding alternative services when benefits are exhausted. + Offer services to members and their families who have acute and complex health care needs, members with chronic conditions at risk for poor health outcomes and members who are terminal and nearing end of life. + Work within the health plan framework of managing medical expenses while also working to improve access and quality care to our members. Qualifications: Required q ualifications for this position include: + Current, unencumbered Oregon Registered Nurse (RN) license + Current valid driver's license + Five years of clinical nursing experience + Experience working with physicians in the collaboration and management of patient care Preferred q ualifications for this position include: + Bachelor's degree in nursing or health education + Washington RN license in addition to Oregon + Current nursing experience in the following areas: cardiology, endocrinology, pediatrics, obstetrics, oncology, respiratory, and/or health education About Providence in Oregon As the largest healthcare system and largest private employer in Oregon, Providence offers exceptional work environments and unparalleled career opportunities. The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
